You can now intimate your loss online by filling this simple form. This facility is introduced for registering your Health (for claims on Reimbursement basis), Hospital cash, Personal Accident and Home claims on-line. Please note that this online intimation is for reimbursement claims only. Our Claims team will contact you to guide you through the process.

Easy Steps for registering your Claim online:
1. Notify your reimbursement claim online by submitting the form below.
2. Download the relevant claim form from our website (or) request for one through our call centre.
3. Claims team will contact you to guide through the claim process. You may also refer to our claim procedures pages under the Claims section of our website.
4. Fill up the claim form and forward to our claims team along with the required documents.

Please note for, any claim under Home Insurance policy, customers are requested to intimate online as well through our call centre. This is required for us to arrange a survey and assess the damages immediately.
