



Personal Accident Policy is a worldwide cover providing protection for the employees against any accidental injuries sustained by the individuals resulting in death and disablement.

The Policy covers bodily injury due to an accident caused by external, violent and visible means and resulting in death/disablement within 12 calendar months from the date of accident.
The cover available is worldwide and for 24 hours. However in case any organization wishes to opt for a restricted cover for their employees for the duration during which they are only on duty, the same can be provided by offering suitable discount in the premium.

Death or bodily injury sustained from
- Intentional Self injury, suicide or even attempted suicide
- While under the influence of intoxicating liquor or drugs
- While engaging in activities associated with aviation or ballooning activities, while
mounting into or dismounting from a balloon and / or an aircraft (other than while
traveling as a passenger)
- Arising out of any criminal activities committed by the insured person
- Death or disablement of any kind resulting directly or indirectly or in consequence of
child birth/pregnancy
- Duly filled claim form
- Copy of FIR duly attested by the issuing authority/Notary Public
- English Translation of all vernacular documents duly attested by Notary Public
- Copy of postmortem duly attested by the issuing authority/Notary Public
- Original death certificate
- Panchanama report/inquest report duly attested by a Notary Public
- Admission/death summary in case the deceased person was admitted to a hospital after
the accident
- Original Legal Heir Certificate
- Duly filled claim form
- Copy of FIR duly attested by the issuing authority
- English Translation of all vernacular documents duly attested by Notary Public
- Admission/discharge summary of the hospital where he was admitted after the accident
- Attending doctor''s certificate clearly specifying whether there is any disablement and
if so, the percentage of disablement (this certificate forms part of the disablement
claim form)
- Copies of x-ray/scan/test reports taken in connection with the accident/treatment
- Original Certificate from the employer confirming period of absence from duty
